ResponsibilitiesPOSITION SUMMARYThe Assessment & Referral Counselor is responsible for facilitating facility admissions by performing initial assessment and referral activities for all in-coming and potential patients, consulting with potential patients, patient families, medical staff and other hospital staff, and counseling patients and patient families with regard to current assessments, recommended referrals and recommended required levels of care.PM Position- Full TimeSunday - Thursday 3pm-11:30pmQUALIFICATIONSMaster's Degree in social work, psychology, counseling or a related field: or equivalent combination of education and experience. A minimum of two (2) years experience in a mental health setting preferred. Working knowledge of the Addiction & Recovery field and experience in clinical interviewing, patient assessment, family motivation, treatment planning, communicating with external review organizations or comparable entities, and working effectively with people of diverse backgrounds.RESPONSIBILITIES/STANDARDS OF PERFORMANCEAdheres to facility standards concerning conduct, dress, attendance, punctuality, and meal & rest breaks.* Maintains confidentiality of facility employees and patient information.* Adheres to facility and department policies and procedures and complies with the Corporate Compliance Program and the Standards of Conduct.* May serve as Qualified Mental Health professional (QMHP).* Maintains proper boundaries when interacting with patients and their families, visitors, professional staff, vendors, and co-workers.* Offers and accepts constructive feedback in peer and supervisory relationships modifies behavior in response to feedback.* Adaptability/Flexibility: Easily adjusts to changes in routine, assignments, and company/client needs.* Demonstrates initiative by voluntarily recommending resourceful, alternative, or original ideas/procedures for work improvement or problem solution.* Demonstrates effective time management skills by prioritizing tasks, anticipating needs, and making adjustments.* Quality of work: Assignments are accurate, complete, and comply with objectives.* Customer Service* Supports facility internal and external customer service standards by demonstrating excellent guest relations in communication, cooperation and in giving assistance to patients, clinicians, co-workers and visitors in a prompt and courteous manner.* Maintains a positive working relationship with co-workers by demonstrating a "team player" approach.* Maintains only appropriate interpersonal communication with other staff members, patients, visitors, and clinicians.* Cultural Diversity* Recognizes that everyone is uniquely multicultural, belonging to many subgroups, which define the person's belief system, behavior, language, work ethic, etc.* Is able to identify instances where one's own cultural, ethnic bias impact decision-making and behavior and makes an effort to reduce these occurrences.* Shows an awareness of differences in communication styles, cultural and spiritual values, and work ethics. Avoids judging others by one's own code of behaviors.* Performance Improvement/Education* Completes competency tests, programs, and self-assessments as required.* Attends all mandatory facility inservices, department meetings, training and staff development activities as required.* Maintains and forwards up-to-date documentation of certifications, registrations and licenses to Human Resources for placement in employment records.* Safety Rules and Procedures/Infection Control* Complies with safety and security policies and procedures, including reporting of incidents and injuries and responding to codes.* Keeps workspace clean and free of work hazards, follows hospital hazard communication program. Maintains equipment in safe working order, reports problems to supervisor and/or maintenance.* Complies with hospital policy regarding identification badge.* Consistently utilizes universal / standard precautions and environmental safety guidelines.* Essential Functions Job Skills* Conducts in-person triage assessment of potential patients, and communicates that assessment to clinicians and nursing staff.* Obtains and records accurate documentation for each call including clinical description, referral sources, demographic information, financial/insurance information and follow-up requirements.* Recommends appropriate referrals based on patient condition, and communicates referrals to the Assessment & Referral Director (or designee) and to the appropriate facility staff.* Counsels patients and families in regard to recommended levels of care, community resources, and financial arrangements.* Responds appropriately to telephone inquiries, providing accurate information, complete pre-admission screening, empathic crisis intervention and determining suitable referral.* Process all requests for information to all referral sources.* Provide customer and requested information and external referrals.* Assists with providing facility tours to physicians and prospective patients and families.* Assist with arranging the admission of patients, notify the business office, begins verification of benefit coverage, and initiates calls to physicians /licensed practitioners as required and complete admission packets.* Obtains the appropriate patient documents and records.* Maintains statistics and other data, and supports the outcome system.* Maintains good communication and relations with the medical staff, treatment coordinator, and other facility staff.* Maintains log of all incoming calls, documents disposition and other appropriate information.* Provides assistance with available community resources to persons inappropriate or unavailable for admission but who are requiring further guidance and referrals.* Assist potential patients in gaining access to federal and state support systems.* Consult with the business office as needed to clarify data and ensure the insurance pre-certification process is complete.* Provides clinical pre-certification information to patients, managed care companies, insurance companies and other third party reviewers to establish the length of stay or number of certified days.* Age-Specific Care* Demonstrates understanding of the developmental stages and can assess developmental milestones.* Children* Adolescent* Adult* Geriatric* Assesses patients for behaviors consistent with the developmental stages and makes note of any deviation in behavior.* Performs age appropriate activity assessments and intervention utilizing the following:* Substance Abuse / use* Sexuality* Education / School* Peer Group Relations* Identity / Self Esteem* Stress coping and leisure lifestyle* Physical Limitations*PHYSICAL, MENTAL, AND SPECIAL DEMANDSReaching: Extending hand(s) and arm(s) in any direction.Walking: Moving about on foot to accomplish tasks, particularly for long distances.Fingering: Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand or arm as in handling.Grasping: Applying pressure to an object with the fingers and palm.Talking: Expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly.Hearing: Perceiving the nature of sounds with no less than a 40db loss @ Hz, 1,000 Hz and 2,000 Hz with or without correction. Ability to receive detailed information through oral communication, and to make fine discriminations in sound, such as when making fine adjustments on machined parts.Repetitive motions: Substantial movements (motions) of the wrist, hands, and/or fingers.Non-Sedentary Work: Exerting up to 100 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
